心电图T波变化情况、血浆N-proBNP水平和Gensini评分对心肌梗死患者预后的预测及各指标的相关性分析

The Changes of ECG T-wave, Plasma N-proBNP Level and Gensini Score on the Prognosis of Patients with Myocardial Infarction and the Correlation Analysis

摘要: 本文选取2017年10月~2019年2月收入我院的132例急性ST段抬高型心肌梗死患者作为研究对象,按照冠脉造影检查结果分为单支病变组(50例)、双支病变组(45例)和三支病变组(37例),比较了三组患者心电图T波变化情况、血浆中N末端脑钠肽前体(N-proBNP)水平和Gensini评分水平以及病变支数与各指标的相关性。检测结果显示三组患者血浆N-roBNP水平、左心室射血分数和Gensini评分组间有显著性差异(P<0.05);单支病变组、双支病变组和三支病变组持续性T波倒置患者数有差异(P<0.05);病变支数与T波变化情况、血浆N-proBNP水平、Gensini评分均呈正相关;多因素逻辑回归分析表明,血浆N-proBNP水平、Gensini评分、年龄是心肌梗死患者发生心功能不全的独立影响因素。这些结果提示心肌梗死患者病变血管支数越多,心电图T波出现持续性倒置的概率越高、血浆N-proBNP水平和GENSINI评分越高,血浆N-proBNP水平和Gensini评分可作为心肌梗死患者预后的预测因子。

Abstract: In this study, 132 patients of myocardial infarction with acute ST-segment elevation in our hospital from October 2017 to February 2019 were selected as research objects. According to the results of coronary angiography, the patients were divided into the single-vessel lesion group (50 cases), the double-vessels lesion group (45 cases) and the three-vessels lesion group (37 cases). The changes of ECG T-wave, N-proBNP level in plasma, Gensini score level were compared in the three groups, as well as the correlation between lesion number and various indicators. The results showed that there were significant differences in the N-proBNP level, left ventricular ejection fraction and Gensini level between the three groups (P<0.05). The differences of continuous T-wave inversion between the groups were statistically significant (P<0.05). The number of lesions was positively correlated with T-wave, N-proBNP level and Gensini score. Multivariate logistic regression analysis showed that N-proBNP level, Gensini score and age were independent influencing factors of cardiac dysfunction in patients with heart infarction. These results suggested that the more pathological vessels in patients with myocardial infarction, the higher changes of ECG T-wave, N-proBNP level and Gensini score. The N-proBNP level in plasma and Gensini score could be used as prognostic factors for patients with myocardial infarction.
