侵蚀性葡萄胎的MRI影像学表现及其诊断效果

摘要: 为探讨侵蚀性葡萄胎(IHM)的磁共振成像(MRI)影像学表现及其诊断效果,本研究对36例疑似IHM患者均行MRI平扫和动态增强扫描(DCE)检查,观察了IHM的MRI影像学表现,分析了MRI诊断IHM的灵敏度、特异度和准确度。结果显示,IHM的MRI影像学表现具有特征性;MRI诊断IHM的灵敏度为75.00%、特异度为83.33%、准确度为77.78%;IHM组的时间-信号强度曲线(TIC)以Ⅰ型为主,良性病变组的TIC以Ⅲ型和Ⅳ型为主,差异有统计学意义(P<0.05)。本文证实MRI检查可清晰显示宫腔内情况及子宫肌层受侵程度,MRI平扫结合DCE可提高诊断IHM的准确性。

Abstract: In order to investigate the magnetic resonance imaging (MRI) features and diagnosis effect in invasive hydatidiform mole (IHM), 36 patients with suspected IHM were examined by MRI plain scan and dynamic contrast enhanced (DCE) in this study. The MRI features of IHM were observed, as well as the sensitivity, specificity and accuracy of MRI in the diagnosis of IHM. The results showed that the MRI features of IHM are characteristic. The sensitivity of MRI in the diagnosis of IHM was 75.00%, the specificity was 83.33%, and the accuracy was 77.78%, respectively. The main type of time-signal intensity curve (TIC) in IHM group was type Ⅰ, the main type of TIC in benign group was type Ⅲ and Ⅳ, the difference was statistically significant (P<0.05). Therefore, MRI can clearly show the condition of uterine cavity and invasion degree of myometrium. MRI plain scan combined with DCE can improve the accuracy in the diagnosis of IHM.
