经阴道三维超声和MRI在子宫内膜癌患者术前评估中的一致性分析

Consistency Analysis of Transvaginal Three-dimensional Ultrasound and MRI in Preoperative Evaluation of Endometrial Cancer

摘要: 为探讨经阴道三维超声(3D-TVS)和磁共振(MRI)在子宫内膜癌患者术前评估中的一致性,选取子宫内膜癌患者356例,术前均接受3D-TVS和MRI检查,分别从肿瘤大小、肌层浸润、宫颈浸润、淋巴结转移和肿瘤分期等方面分析两种诊断方式进行子宫内膜癌术前评估的一致性。结果发现,3D-TVS和MRI术前评估肿瘤上下径、前后径、左右径的ICC均大于0.7,一致性较好;术前评估肌层浸润、宫颈浸润、淋巴结转移、肿瘤分期的P值均小于0.05,提示各项Kappa值一致性均存在,评估宫颈浸润的Kappa值为0.913,具有高度一致性。由此可知,3D-TVS和MRI在子宫内膜癌患者术前评估中一致性较强,均可作为术前评估手段为手术医师提供参考。

Abstract: In order to study the consistency of transvaginal three-dimensional ultrasound (3D-TVS) and magnetic resonance imaging (MRI) in the patients with endometrial carcinoma, 356 cases of endometrial carcinoma were selected as subjects. All patients accepted 3D-TVS and MRI before surgery. The consistency of preoperative assessment in endometrial cancer of these two methods were analyzed respectively from the tumor size, muscle layer infiltration, cervical infiltration, lymph node metastasis and tumor stage and so on. The results showed that the ICC of 3D-TVS and MRI on tumor diameter was less than 0.7, which indicated good consistency. The P values of preoperative evaluation of 3D-TVS and MRI on muscular layer infiltration, cervical infiltration, lymph node metastasis and tumor stage were all less than 0.05, suggesting that all Kappa values were consistent. The Kappa value of cervical infiltration was 0.913, showing a high consistency. In conclusion, 3D-TVS and MRI showed strong consistency in preoperative evaluation of patients with endometrial cancer, and both of them could provide effective reference for surgeons.
