经腹彩超联合血清HCG、AFP检查对凶险型前置胎盘合并胎盘植入的诊断价值

The Value of Transabdominal Ultrasound Combined with HCG and AFP in the Diagnosis of Pernicious Placenta Previa with Placenta Accreta

摘要: 本研究探讨经腹彩超(TAUS)联合血清人绒毛膜促性腺激素(HCG)、甲胎蛋白(AFP)检查在凶险型前置胎盘(PPP)合并胎盘植入(PA)诊断中的应用价值。将73例PPP患者分为胎盘植入组(31例)和无胎盘植入组(42例),35例健康孕妇作为对照组,产前均行TAUS和血清HCG、AFP检查。结果显示,胎盘植入组HCG、AFP水平均高于无胎盘植入组和对照组(P<0.05)。TAUS联合血清HCG、AFP检查诊断PPP合并PA的AUC、敏感度、特异度分别为0.912、96.8%、85.7%,均高于单一检查(P<0.05)。因此,血清HCG、AFP诊断PPP合并PA均有一定价值,联合TAUS可提高诊断效能。

Abstract: The application value of transabdominal ultrasound (TAUS) combined with human chorionic gonadotropin (HCG) and alpha fetoprotein (AFP) in the diagnosis of pernicious placenta previa (PPP) with placenta accreta (PA) is analyzed in this study. 73 patients with PPP were divided into placental implantation group (31 cases) and non placental implantation group (42 cases), while 35 cases healthy pregnant women as control group. The levels of HCG and AFP and TAUS were examined before delivery. The results showed that the levels of HCG and AFP in placental implantation group were higher than those of non placental implantation group and control group (P<0.05). The AUC, sensitivity and specificity of TAUS combined with HCG and AFP in the diagnosis of PPP with PA were 0.912, 96.8% and 85.7%, respectively, which were higher than those of single test (P<0.05). Therefore, it has a certain value of HCG and AFP in the diagnosis of PPP with PA, and combination with TAUS can improve the diagnosis efficiency.
