多层螺旋CT小肠造影对炎症性肠病的诊断价值

Value of Multi-slice Spiral Computed Tomographic Enterography in the Diagnosis of Inflammatory Bowel Disease

摘要: 探讨多层计算机断层扫描肠造影(CTE)在炎症性肠病(IBD)中的诊断及病情评估价值。研究纳入临床疑为IBD患者150例,最终确诊102例,分析CTE对IBD的临床诊断及疾病评估价值,并以结肠镜结果为参照,分析其对病变肠段定位的准确性。结果发现克罗恩病(CD)及溃疡性结肠炎(UC)患者肠壁增厚、肠壁异常强化、肠道狭窄、肠系膜血管增生等发生率较普通肠炎高(P<0.05),其中肠壁异常强化、肠壁增厚对IBD的诊断效能较高;CTE与肠镜检查相比,对CD、UC病变部位检测具有较高的一致性(P<0.001);活动期患者CTE显示肠壁较厚、分层强化、肠系膜血管增生较多见(P<0.05)。因此,CTE对IBD有良好的诊断价值,对CD的活动性评价具有一定的临床价值。

Abstract: The purpose of this study is to explore the value of multi-slice spiral computed tomography enterography (CTE) in the diagnosis and evaluation of inflammatory bowel disease (IBD). A total of 150 patients with clinically suspected IBD were included in the study, and 102 of them were clinically confirmed. The clinical diagnosis value of CTE for IBD was analyzed. Compared with the results of colonoscopy, the accuracy of positioning the diseased bowel segment was analyzed. In patients with Crohn's disease (CD) and ulcerative colitis (UC), the intestinal wall thickened, abnormally strengthened, stenosis and mesenteric vascular proliferation was more common than ordinary enteritis, of which the intestinal wall was abnormally strengthened and thickening of the intestinal wall had high diagnostic efficiency for IBD.CTE and colonoscopy had a high consistency in the detection of CD and UC lesions (P<0.001).The CTE showed that thick intestinal wall, stratified enhancement, and mesenteric vascular proliferation was more common in the active stage patients (P<0.05). CTE has good diagnostic value for IBD, can accurately locate the diseased intestinal segment, and has good clinical value for the evaluation of CD activity.
