超声对患儿肺部感染的诊断价值及与血清PCT、PCIS评分的相关性

The Diagnostic Value of Ultrasonography for Pulmonary Infection and Its Correlation with Serum PCT and PCIS Scores

摘要: 本文探讨肺部超声诊断肺部感染患儿的价值及与血清降钙素原(PCT)、小儿危重病例评分(PCIS)的相关性。选取肺部感染患儿120例作为感染组,选取同期健康小儿120例作为对照组,两组均接受肺部超声检查。感染组LUS评分、血清PCT均显著高于对照组且差异具有统计学意义(P<0.05),感染组的PCIS评分显著低于对照组(P<0.05);经简单线性相关分析,感染组患儿的LUS评分与血清PCT呈显著正相关关系(r=0.704、P<0.05),感染组患儿LUS评分与PCIS评分呈显著负相关关系(r=-0.742、P<0.05);LUS评分诊断肺部感染患儿的灵敏度为82.76%、特异度为76.60%,ROC曲线下面积为0.845。肺部超声诊断肺部感染患儿具有较高的临床价值,同时与PCIS评分、PCT具有较好的相关性,该检查方法无放射性,安全可靠。

Abstract: This article exploreed the value of pulmonary ultrasound in the diagnosis of children with pulmonary infection and its correlation with serum procalcitonin (PCT) and pediatric critical illness score (PCIS). A total of 120 children with pulmonary infection (infection group) and 120 healthy children (control group) who underwent child care during the same period were selected. Both groups received pulmonary ultrasound examination. The LUS score and serum PCT of the infection group were significantly higher than those of the control group (P<0.05), and the PCIS score of the infection group was lower than that of the control group (P<0.05). After simple linear correlation analysis, in the infection group there was a significant positive correlation between the LUS score and serum PCT (r=0.704, P=0.000<0.05), and the LUS score and PCIS score were significantly negatively correlated (r=-0.742, P=0.000<0.05). The sensitivity of LUS score to diagnose children with pulmonary infection was 82.76%, specificity was 76.60%, and the area under the ROC curve was 0.845. Pulmonary ultrasound diagnosis of children with pulmonary infection has high clinical value, and has a good correlation with PCIS score and PCT. This examination method has benefits of being non-radioactive, safety and reliability.
