床旁肺脏超声评分在诊治呼吸窘迫综合征早产儿中的应用

Application of Bedside Lung Ultrasound Score in the Diagnosis and Treatment of Premature Infants with Respiratory Distress Syndrome

摘要: 本文探讨床旁肺脏超声评分在呼吸窘迫综合征早产儿诊治中的价值。选取88例呼吸窘迫综合征早产儿作为观察组,同时选取100例无呼吸窘迫综合征早产儿作为对照组,比较两组床旁肺脏超声评分(LUS)差异,同时比较观察组不同临床资料患儿LUS评分差异,分析LUS评分预测呼吸窘迫综合征并发支气管肺发育不良、极危重症呼吸窘迫综合征的价值。观察组双肺、左肺和右肺LUS评分明显高于对照组(P<0.05)。观察组有支气管肺发育不良、呼吸窘迫评分>8分、极危重症(新生儿危重病例评分<70分)患儿双肺LUS评分明显高于其他患儿(P<0.05)。双肺LUS评分与呼吸窘迫评分呈正相关(P<0.05),与新生儿危重病例评分呈负相关(P<0.05)。床旁高频超声监测在呼吸窘迫综合征早产儿并发支气管肺发育不良、疾病程度诊治中有一定应用价值。

Abstract: This paper discussed the value of bedside lung ultrasound score in the diagnosis and treatment of premature infants with respiratory distress syndrome. 88 premature infants with respiratory distress syndrome treated were selected as the observation group, and 100 premature infants without respiratory distress syndrome were selected as the control group. Compared the differences of bedside lung ultrasound score (LUS) between the two groups, and each different clinical data in the observation group, further analyzed the value of LUS score in predicting respiratory distress syndrome complicated with bronchopulmonary dysplasia and extremely critical respiratory distress syndrome. The LUS scores of bilateral lung, left lung and right lung in the observation group were significantly higher than those in the control group (P<0.05). In the observation group, the lung LUS scores of children with bronchopulmonary dysplasia, respiratory distress score > 8, and extremely critical disease (neonatal critical case score <70) were significantly higher than those of other children (P<0.05). Bilateral lung LUS score was positively correlated with respiratory distress score (P<0.05), and negatively correlated with neonatal critical case score (P<0.05). Bedside high frequency ultrasound monitoring has certain application value in the diagnosis and treatment of premature infants with respiratory distress syndrome complicated with bronchopulmonary dysplasia and disease degree.
