光学相干断层扫描联合眼部B超在白内障术前眼底病检查中的应用

The Application of Optical Coherence Tomography Combined with B-ultrasound in the Examination of Fundus Diseases in Patients with Cataract

摘要: 本文探讨了光学相干断层扫描(OCT)联合眼部B超在白内障术前眼底病检查中的应用。选取白内障患者220例(共326眼),所有患者术前均给予眼压、视力、裂隙灯等常规检查,并接受眼部B超和OCT检查,比较不同检查方式对白内障术前眼底病的检出情况。结果显示:OCT检出59眼有眼底病,检出率为18.10%;眼部B超检出23眼有眼底病,检出率为7.06%;OCT联合眼部B超检出66眼有眼底病,检出率为20.25%。其中,眼部B超检出了OCT未检出的视网膜囊肿及漏诊的视网膜脱离;而OCT检出了B超难以检出的黄斑视网膜前膜、黄斑裂孔、黄斑水肿、中心性浆液性脉络视网膜病变、玻璃体黄斑牵拉等眼底病。OCT联合眼部B超对白内障术前眼底病的检出率均高于OCT、眼部B超单项检查,差异均有统计学意义。因此,OCT联合眼部B超检查可提高白内障术前眼底病的诊断准确率。

Abstract: The optical coherence tomography (OCT) combined with B-ultrasound were applied in the examination of fundus diseases in patients with cataract in this study. 220 patients (326 eyes) with cataract were given routine examinations such as intraocular pressure, visual acuity and slit lamp, as long as B-ultrasound and OCT examination. The detection of fundus diseases with different examination methods was compared. The results show that, 59 eyes of fundus diseases were detected by OCT, the detection rate was 18.10%; 23 eyes of fundus diseases were detected by B-ultrasound, the detection rate was 7.06%; 66 eyes of fundus diseases were detected by OCT combined with B ultrasound, the detection rate was 20.25%. Retinal cyst and retinal detachment were detected by B-ultrasound, which was OCT did not find and failed to diagnose. OCT also detected macular epiretinal membrane, macular hole,macular edema, central serous chorioretinopathy, vitreous macular traction, which were hard for 892 B-ultrasound to detected. The detection rate of OCT combined with B-ultrasound was significantly higher than that of the OCT or B-ultrasound alone. In a word, OCT combined with B-ultrasound can improve the diagnostic efficacy for fundus diseases and predict the recovery of postoperative vision.
