盆底超声参数联合血清25(OH)D水平检测对产后压力性尿失禁的预测价值及与病情关系

Predictive Value of Pelvic Floor Ultrasound Parameters Combined with Serum 25(OH)D Level on Postpartum Stress Urinary Incontinence and the Relationship with the State of Disease

摘要: 选取120例产后压力性尿失禁(PSUI)患者为研究组,同期120例非PSUI产妇为对照组。盆底超声及实验室检测发现,研究组的尿道旋转角度(URA)、膀胱尿道后角(PUVA)、膀胱颈移动度(BND)水平高于对照组,血清25羟维生素D[25(OH)D]水平低于对照组(P<0.05)。URA、PUVA、BND和血清25(OH)D联合预测PSUI的AUC为0.859,敏感度为77.50%,特异度为84.17%。PSUI患者的ICI-Q-SF评分与URA、PUVA、BND水平呈正相关,与血清25(OH)D水平呈负相关(P<0.05)。Logistic回归分析显示URA、PUVA、BND和血清25(OH)D为PSUI发生的重要影响因素。因此本文可证实盆底超声参数URA、PUVA、BND及血清25(OH)D水平与患者病情程度密切相关,在预测PSUI发生风险方面具有较高应用价值。

Abstract: 120 patients with postpartum stress urinary incontinence (PSUI) were selected as the research group, and 120 non-PSUI women in the same period were selected as the control group. Pelvic floor ultrasound and laboratory tests found that the pelvic floor ultrasound parameters in the research group including urethral rotation angle (URA), bladder urethral posterior angle (PUVA), bladder neck mobility (BND) were higher than those of the control group, while serum 25-hydroxyvitamin D[25(OH)D] level was lower than that of the control group (P<0.05). The combined prediction of URA, PUVA, BND and serum 25(OH)D for PSUI was 0.859, and the sensitivity was 77.50%, specificity was 84.17%. The ICI-Q-SF scores of PSUI patients were positively correlated with URA, PUVA, BND and negatively correlated with serum 25(OH)D level (P<0.05). URA, PUVA, BND, and serum 25(OH)D were important factors influencing the occurrence of PSUI (P<0.05). In a word, the pelvic floor ultrasound parameters URA, PUVA, BND and serum 25(OH)D level are closely related to the state of illness, and have high application value in predicting PSUI.
