孕早期、孕中期产前超声对胎儿中枢神经系统畸形的筛查价值

The Screening Value of Prenatal Ultrasound for Fetal Central Nervous Malformation in Early and Mid Pregnancy

摘要: 本文分析了孕早期、孕中期产前超声对胎儿中枢神经系统畸形(CNSM)的筛查价值。选取孕妇2298例为研究对象,所有孕妇均于孕早期(孕11~13+6周)、孕中期(孕14~27+6周)给予产前超声检查。产后超声或病理检查显示,胎儿CNSM有200例(8.70%);孕早期联合孕中期超声对胎儿CNSM检出符合率明显高于孕早期超声(P<0.05);孕早期超声筛查胎儿CNSM的敏感度为78.00%、特异度为91.61%、准确度为90.43%,孕早期联合孕中期超声筛查的敏感度为92.00%、特异度为94.47%、准确度为94.26%,效果明显好于孕早期筛查(P<0.05)。本文证实了胎儿CNSM的产前超声筛查可提早至孕早期,有利于及早筛查胎儿CNSM,但仍需孕中期超声进行有效补充。

Abstract: The value of prenatal ultrasound in screening for fetal central nervous system malformations (CNSM) in early pregnancy and mid pregnancy was analyzed. 2298 pregnant women were selected as the subjects of this study, who were given prenatal ultrasound examination in early pregnancy (11-13+6 weeks) and mid pregnancy (14-27+6 weeks). The postpartum ultrasound or pathological examination showed that there were 200 cases (8.70%) of fetal CNSM. The coincidence rate of fetal CNSM in early pregnancy combined with mid pregnancy ultrasound was significantly higher than that in early pregnancy (P<0.05). The sensitivity, specificity and accuracy of early pregnancy combined with mid pregnancy ultrasound was significantly higher than those of early pregnancy (P<0.05). This study confirms that the prenatal ultrasound screening of fetal CNSM can be brought forward to early pregnancy, which is beneficial to early screening of fetal CNSM. But in order to have better screening effect, it still needs to be supplemented by ultrasound in mid pregnancy.
