肺癌是世界上发病率和死亡率最高的恶性肿瘤,其中85%的病理类型是非小细胞肺癌[1]。非小细胞肺癌5年生存率为15%左右,并且在近30年来没有明显增加。近年来的研究表明吸烟、被动吸烟、嗜酒、空气污染等因素是肺癌的主要诱因。肺癌的治疗方法多样,如手术、化疗、放疗、分子靶向治疗、免疫治疗,但是其死亡率仍逐年上升,关键问题在于大部分患者被诊断为肺癌时已处于疾病晚期阶段[2]。Tang等[3]研究发现,肺癌的临床病理特征,如肿瘤大小、TNM分期、淋巴结转移等与非小细胞肺癌患者的预后相关,可见评估这些特征可以为预测非小细胞肺癌的预后提供重要信息。
Xu等曾想通过低剂量CT发现早期肺癌,进而提高肺癌患者预后,但结果发现这种方法仍然会造成误诊和漏诊[4],因此,需要新的技术手段提高早期肺癌诊断率。与普通CT相比,增强CT能够提供更稳定的结果,并且对良恶性肿瘤的判断更加准确[5],此外,增强CT对识别淋巴结分化程度具有更强的特异性。在肺癌的CT扫描中,增强CT是评估淋巴结转移常用的影像学技术[6],与普通CT相比,增强CT能够全面评估肿瘤大小、淋巴结转移及淋巴结大小,是医学诊疗中常用的评估非小细胞肺癌临床分期的影像学手段[7-9]。然而,有关增强CT与非小细胞肺癌临床病理学特征及预后的研究非常有限,因此,本研究对增强CT与非小细胞肺癌患者临床病理学特征及预后的关系进行分析,探索增强CT在评估非小细胞肺癌预后中的潜在价值。
1 资料与方法 1.1 一般资料选取2015年2月~2016年7月的150例于秀山土家族苗族自治县人民医院行胸部增强CT的非小细胞肺癌患者为研究对象,其中,男性98例、女性52例;年龄31~80岁,46名患者小于60岁,104名大于60岁。150名患者中,肺腺癌患者77名、鳞癌患者63例、腺鳞癌患者10例。根据CT表现,中枢性肺癌56名、周围性肺癌94名。根据肿瘤分化程度,低分化68例、中度分化45例、高度分化37例。65名患者有淋巴结转移、85名患者无淋巴结转移。
纳入标准:①明确的病理诊断和完整的病历资料;②在我院行胸部增强CT检查;③术前没有接受过化疗、放疗或免疫治疗等肿瘤相关治疗;④无其他器质性合并疾病,凝血功能正常。排除标准:①病理结果不明确或病历资料不完整;②对自己疾病缺乏正确认知;③合并严重的器质性并发症或合并其他部位肿瘤;④拒绝参与此项研究。所有参与者对本次研究知情并签署知情同意书。
1.2 增强CT扫描方法飞利浦华晨64层螺旋CT扫描仪(美国GE Optima 64排)用于胸部扫描,参数设置为120 kV,250 mA,厚度为5 mm。
当患者屏住呼吸或平静呼吸时从肺尖部开始扫描。将造影剂(碘帕多370,德国拜耳公司,1.5 mL/kg)以3 mL/s的速度注入尺静脉。记录最大肿瘤层中距离肺门最近和最远的边距,并计算其平均值。根据肿瘤直径, 分为≤3 cm组、3~7 cm组和≥7 cm组。记录病变区域单级螺旋扫描的最大CT增强值与同一层中的普通CT值,根据该值,<15 HU被认为是略有增强,≥15 HU为显着增强。同时扫描肺门和纵隔淋巴结,根据扫描结果,将患者分为无淋巴结肿大组、肺门淋巴结肿大组和纵隔淋巴结肿大组。根据淋巴结直径,将患者分为≥1 cm组和<1 cm组。
1.3 随访使用问卷调查或电话随访的方式调查参与研究患者的基线信息和生存状况,随访时间由病理诊断日截止到2018年8月或死亡。随访时间为3年。
1.4 统计方法所有数据分析均使用SPSS 21.0统计学软件进行处理。卡方检验用于分析肿瘤大小、增强CT值及淋巴结肿大与病理特征关系。对数秩和检验比较不同组的生存率。Cox比例风险模型和logistic回归分析被用于确定独立的预后危险因素。P < 0.05认为具有统计学意义。
2 结果 2.1 非小细胞肺癌患者的增强CT参数150例非小细胞肺癌患者的增强CT结果如下:63例肿瘤直径≤3 cm,3~7 cm为36例,≥7 cm为51例。根据CT强化程度,84例患者无强化,66例患者有强化。根据淋巴结肿大情况,发现无淋巴结肿大20例,60例存在肺门淋巴结肿大,70例存在纵隔淋巴结肿大。根据淋巴结大小,87名患者的淋巴结 < 1 cm,63名患者淋巴结≥1 cm。
2.2 增强CT参数与非小细胞肺癌患者病理特征之间的关系卡方检验结果表明,分化程度、TNM分期和淋巴结转移均与肿瘤大小、CT强化值、淋巴结肿大和淋巴结大小相关(P < 0.05)。但是,病理类型与肿瘤大小、CT强化值、淋巴结肿大和淋巴结大小无关(P> 0.05),详见表 1。
对数秩和检验发现,肿瘤大小、CT强化值、淋巴结肿大和淋巴结大小均与非小细胞肺癌患者的预后显著相关(P < 0.05)(见表 2)。Cox比例风险模型显示,淋巴结转移、肿瘤大小、淋巴结肿大、淋巴结大小和CT强化值是非小细胞肺癌预后的独立危险因素(表 3)。
以患者的生存状况为因变量,CT强化值、肿瘤大小、淋巴结肿大和淋巴结大小作为自变量,进行多元logistic回归分析(表 4),发现肿瘤强化、较大的肿瘤、淋巴结肿大和转移是影响非小细胞肺癌患者预后的危险因素。
本研究通过对非小细胞肺癌患者进行肺部增强CT扫描,研究增强CT参数与NSCLC患者的临床病理学特征及预后之间的关系,结果显示,非小细胞肺癌患者的临床病理特征和预后与肿瘤大小、CT强化值、淋巴结肿大及淋巴结大小等增强CT参数显著相关,表明增强CT在评估非小细胞肺癌患者预后中具有潜在价值。增强CT被认为是通过评估肿瘤大小、淋巴结肿大等预测有无局部淋巴结转移的重要影像学方法[13, 14],包括肿瘤大小和淋巴结肿大在内的临床病理学特征可以影响非小细胞肺癌患者的预后[15]。有研究表明,在肺癌患者中,肿瘤大小与TNM分期、淋巴结转移等临床病理学特征显著相关,并且肿瘤大小还与5年生存率和预后密切相关[10-12]。此外,Tsutani等[16]研究发现,不同肿瘤大小的患者其临床病理学特征和预后显著不同,表明肿瘤大小可能与肿瘤的恶性等级相关。同时,Stojiljkovic等[17]发现,较大的肿瘤可以促进肿瘤进展至更高的TNM分期(Ⅱ或Ⅲ),并导致纵隔淋巴结受累和隐匿的纵隔转移。而Kang等[18]研究发现,大于2 cm的肿瘤发生淋巴结转移的可能性是肿瘤小于2 cm患者的2倍,更有学者认为,对肿瘤大小和淋巴结大小的评估可有效预测淋巴结转移[19],而且,较大的肿瘤可能与淋巴结转移密切相关,是恶性肿瘤淋巴结转移的主要途径[20]。除了肿瘤大小和淋巴结大小这两个增强CT参数之外,CT发现肿瘤强化意味着肿瘤中存在大量的可能与淋巴结转移相关的血液供应[21]。同时,淋巴结肿大,特别是肺门和纵隔的淋巴结肿大可能与肺癌的淋巴结转移有关[22]。本研究结果表明,肿瘤大小、CT强化值、淋巴结肿大和淋巴结大小与非小细胞肺癌患者临床病理特征具有相关性,此外,分化程度、TNM分期,以及淋巴结转移与非小细胞肺癌患者预后相关,因此,可以认为增强CT是预测非小细胞肺癌患者预后的重要影像学手段。
