锥形束CT在Er:YAG激光联合GTR治疗下颌磨牙根分叉病变效果评估中的应用

摘要: 本文探讨了锥形束CT对Er:YAG激光联合引导组织再生术(GTR)治疗下颌磨牙根分叉病变效果的评估作用。将下颌磨牙Ⅱ度根分叉患者30例分为C组(采用单纯GTR治疗)和L组(采用Er:YAG激光联合GTR治疗),对比了两组术后龈沟出血指数(SBI)、牙龈退缩(GR)、角化龈宽度(KG)、牙周探诊深度(PPD)、水平附着丧失(CAL-H)、垂直附着丧失(CAL-V)等临床指标及锥形束CT测量垂直骨丧失(BL-V)和水平骨丧失(BL-H)的差异。结果显示,两组手术后1年,SBI均减小(P<0.05),GR均增大(P<0.05);PPD、CAL-H、CAL-V较术前均有下降(P<0.05),但L组变化程度大于C组(P<0.05);BL-V、BL-H均明显改善(P<0.05),且两组改善幅度有统计学差异(P<0.05)。本文结果证实,Er:YAG激光联合GTR治疗下颌磨牙根分叉病变的效果优于GTR单用,锥形束CT在对Er:YAG激光联合GTR治疗根分叉病变效果的评估中有一定应用价值。

Abstract: In this paper, we discussed the effect of cone beam CT in assessing the therapeutic effects of Er:YAG laser with guided tissue regeneration (GTR)on furcation involvements of mandibular molars. 30 patients with furcation involvement of mandibular molars (degree Ⅱ) were divided randomly into group C (treated with GTR alone) and group L (treated with Er:YAG laser combined with GTR). Sulus bleeding index (SBI), gingival recession (GR), keratinized gingival (KG), probing pocket depth (PPD), horizontal clinical attachment loss (CAL-H), vertical clinical attachment loss(CAL-V) and cone beam CT measurement indexes such as vertical bone loss (BL-V), horizontal bone loss (BL-H) were observed and compared between the two groups. The results showed that SBI decreased and GR increased in both groups (P<0.05) one year after surgery. PPD, CAL-H and CAL-V decreased in both groups (P<0.05),the changes of group L were greater than those of group C (P<0.05). There were no statistical differences in the pre-operative conical beam CT measurement indexes such as BL-V and BL-H in the two groups (P>0.05). BL-V and BL-H improved one year after operation in both groups (P<0.05), and there were statistical differences in the improvements between the two groups (P<0.05). The results indicate that Er:YAG laser combined with GTR is better than GTR alone in the treatment of furcation involvements of mandibular molars. Cone beam CT has certain application value in assessing the therapeutic effects of Er:YAG laser combined with GTR in the treatment of furcation defect.
