3D-PDU联合血清检测中央型前置胎盘并胎盘植入价值分析

Analysis of the Value of 3D-PDU Combined with Serum to Detect Central Placenta Previa and Placenta Accreta

摘要: 本研究探讨三维能量多普勒超声(3D-PDU)联合血清检测中央型前置胎盘并胎盘植入的价值。选取52例中央型前置胎盘(CPP)并胎盘植入(PA)患者为PA组,选取52例同期无PA的CPP患者为无PA组,同时选取52例超声检查显示胎盘附着在子宫前壁的正常孕妇为对照组。三组均进行3D-PDU检查,并检测血甲胎蛋白(AFP)、可溶性血管内皮生长因子受体1(SFlt-1)水平。结果显示,PA组VI、FI、VFI、血清AFP水平高于无PA组和对照组,血清SFlt-1水平低于无PA组和对照组(P<0.05);VI、FI、VFI、血清AFP、SFlt-1联合诊断CPP并PA的AUC大于各指标单独诊断的AUC,联合诊断的敏感度、特异度分别为90.59%、76.47%;VI、FI、VFI、血清AFP高表达者PA发生率高于低表达者,SFlt-1低表达者PA发生率高于高表达者(P<0.05)。采用3D-PDU联合AFP、SFlt-1水平诊断,有望为临床CPP并PA诊治提供一种有效途径。

Abstract: This study investigated the value of three-dimensional power Doppler ultrasound (3D-PDU) combined with serum in detecting central placenta previa and placenta implantation. Fifty-two patients with central placenta previa (CPP) and placenta accreta (PA) were selected as the PA group, and 52 patients with CPP without PA during the same period were selectedas the PA group. Ultrasound examination of 52 normal pregnant women with placenta attached to the anterior uterine wall was the control group. All were examined by 3D-PDU, and the levels of blood alpha-fetoprotein (AFP) and soluble vascular endothelial growth factor receptor 1 (SFlt-1) were detected. After analysis, it was found that the levels of VI, FI, VFI, and serum AFP in the PA group were higher than those in the PA-free group and the control group, and the SFlt-1 levels were lower than those in the PA-free group and the control group (P<0.05). The AUC of VI, FI, VFI, AFP and SFlt-1 combined to diagnose CPP and PA was greater than the AUC diagnosed by each index alone, and the sensitivity and specificity of the combined diagnosis are 90.59% and 76.47%, respectively. The incidence of PA in patients with high expression of VI, FI, VFI, and serum AFP is higher than that of 269those with low expression, and SFlt-1 is low The incidence of PA in expressers was higher than that in high expressers (P<0.05). The use of 3D-PDU combined with blood AFP and SFlt-1 level diagnosis is expected to provide an effective way for clinical diagnosis and treatment of CPP and PA.
