超声诊断原发性高血压对梗阻性肥厚型心肌病患者心功能的影响

The Effect of Ultrasound Diagnosis of Essential Hypertension on Cardiac Function in Patients with Hypertrophic Obstructive Cardiomyopathy

摘要: 本研究探讨原发性高血压对梗阻性肥厚型心肌病患者左室重构及左心衰竭的影响。选取梗阻性肥厚型心肌病患者140例,其中合并原发性高血压者54例,均给予采用超声心动图检查。合并高血压患者左室舒张末期内径、收缩末期内径明显高于未合并高血压患者(P<0.05),而室间隔厚度、左室射血分数明显低于未合并高血压患者(P<0.05)。合并高血压患者左心衰竭发生比例明显高于未合并高血压患者(P<0.05)。Logistic回归分析显示:年龄、收缩压、左室射血分数是梗阻性肥厚型心肌病伴高血压患者发生左心衰竭的影响因素(OR=1.759、2.273和0.576,P<0.05)。原发性高血压可加重梗阻性肥厚型心肌病患者左室重构,患者发生左心衰竭风险增高。

Abstract: This study investigated the effect of essential hypertension on left ventricular remodeling and left heart failure in patients with hypertrophic obstructive cardiomyopathy. 140 patients with hypertrophic obstructive cardiomyopathy were selected, among them 54 patients were complicated with essential hypertension, and all patients were examined by echocardiography. The left ventricular end diastolic diameter and end systolic diameter in patients with hypertension were significantly higher than those in patients without hypertension (P<0.05), while the ventricular septal thickness and left ventricular ejection fraction were significantly lower than those in patients without hypertension (P<0.05). The incidence rate of left heart failure in patients with hypertension was significantly higher than that in patients without hypertension (P<0.05). Logistic regression analysis showed that the age, systolic blood pressure and left ventricular ejection fraction were the influencing factors of left heart failure in patients with hypertrophic obstructive cardiomyopathy with hypertension (OR=1.759, 2.273 and 0.576, P<0.05). Essential hypertension can aggravate left ventricular remodeling in patients with hypertrophic obstructive cardiomyopathy, and have an increased risk of left heart failure.
