RT-3DE在LVEF降低的心力衰竭患者心室重构中的应用

Application of RT-3DE in Ventricular Remodeling in Heart Failure Patients with Reduced LVEF

摘要: 本文探讨单心动周期全容积实时三维超声心动图(RT-3DE)观察左室射血分数(LVEF)降低的心力衰竭患者左室重构情况。选取110例LVEF减低的心力衰竭患者作为观察组,同时选取50例体检健康者作为对照组,均给予RT-3DE检查,观察两组心室参数。结果显示,观察组右心室舒张末期容积指数(RVEDVI)、右心室收缩末期容积指数(RVESVI)和右心室基底部长径(RVLD)等明显高于对照组(P<0.05),而LVEF明显低于对照组(P<0.05);观察组肺动脉高压(PH)水平≥80 mmHg患者RVEDVI、RVESVI和RVLD明显高于其他患者(P<0.05),而LVEF明显低于其他患者(P<0.05);PH与RVEDVI、RVESVI、LVEF之间有相关性(r=0.454、0.420、-0.331,P<0.05)。LVEF减低的心力衰竭患者PH对患者心室重构有一定影响,值得进一步研究。

Abstract: This paper discussed the left ventricular remodeling in heart failure patients with reduced left ventricular ejection fraction (LVEF) by real-time three-dimensional echocardiography (RT-3DE). 110 patients with heart failure with reduced LVEF were selected as the observation group and 50 healthy people were selected as the control group. They all underwent RT-3DE, and observed the ventricular parameters of the two groups. The results showed the right ventricular end diastolic volume index (RVEDVI), right ventricular end systolic volume index (RVESVI) and right ventricular basal long diameter (RVLD) were significantly higher than that in the control group (P<0.05), while the LVEF was significantly lower than that in the control group (P<0.05). The RVEDVI, RVESVI and RVLD of patients with pulmonary hypertension (PH) ≥ 80 mmHg were higher than those of other patients (P<0.05) in the observation group, while LVEF was lower than that of other patients (P<0.05). There had correlations between PH and RVEDVI, RVESVI and LVEF (r=0.454, 0.420 and -0.331, P<0.05). PH in patients with heart failure with reduced LVEF has a certain effect on ventricular remodeling, which is worthy of further study.
