瞬时弹性成像技术联合血NLR、PDW对慢性HBV感染相关肝硬化预后的预测价值及与肝功能的关联性研究

The Value of Fibroscan Combined with Blood NLR and PDW in Predicting the Prognosis of Liver Cirrhosis Caused by Chronic HBV Infection and Its Relationship with Liver Function

摘要: 本文分析了瞬时弹性成像技术(FibroScan)联合血NLR、PDW对慢性HBV感染相关肝硬化预后的预测价值,并研究了其与肝功能的关系。选取唐山市传染病医院慢性HBV感染相关肝硬化患者86例进行回顾性分析,按患者预后有无肝硬化并发症分为并发症组(n=48)、无并发症组(n=38)。测定对比两组患者FibroScan的肝脏/脾脏硬度值和血NLR、PDW、血清肝功能指标(AST、ALT、TBIL)水平,探讨了肝脏/脾脏硬度值、血NLR、PDW水平与患者肝功能关系,观察肝脏/脾脏硬度值、血NLR、PDW对预后的预测效果。结果可见,并发症组患者的肝脏/脾脏硬度值、血NLR、PDW水平与患者血清AST、ALT、TBIL水平呈正相关,且均高于无并发症组(P<0.05);肝脏/脾脏硬度值、血NLR、PDW预测患者预后出现并发症的价值均较高,联合预测时敏感度为83.33%,特异度为81.58%。本研究证实,FibroScan联合血NLR、PDW预测慢性HBV感染相关肝硬化预后价值较高,且与患者肝功能密切相关。

Abstract: In this paper, we analyzed the predictive value of FibroScan combined with blood NLR and PDW on the prognosis of liver cirrhosis caused by chronic HBV infection and its relationship with liver function. We selected 86 patients with chronic HBV infection-related cirrhosis for retrospective analysis, and they were divided into the complication group (n=48) and the non-complication group (n=38) according to the prognosis of patients with or without cirrhotic complications. The hardness values of liver and spleen, levels of blood NLR/PDW, serum liver function indexes (AST, ALT, TBIL) of FibroScan in the two groups were measured and compared. The relationship between hardness values of liver and spleen, blood NLR/PDW levels and liver function of patients were explored, and the predictive value of liver and spleen hardness values, blood NLR/PDW on prognosis were observed. The results showed that the hardness values of liver and spleen, blood NLR/PDW and serum AST, ALT, TBIL levels in the complication group were higher than those in the non-complication group (P<0.05); the hardness values of liver and spleen, blood NLR/PDW levels were positively correlated with serum AST, ALT, TBIL levels of patients (P<0.05). The efficacy of hardness values of liver and spleen in predicting the prognosis of patients with complications were higher, as well as the blood MLR/PDW. The sensitivity of joint prediction was 83.33%, and the specificity was 81.58%. It can be seen that FibroScan combined with blood NLR/PDW has a higher value in predicting the prognosis of cirrhosis associated with chronic HBV infection, and is closely related to the liver function of patients.
