MRI评价脑卒中伴T2DM患者颈动脉粥样硬化斑块的研究

Study of Carotid Atherosclerotic Plaque in Stroke Patients with T2DM by MRI

摘要: 本文目的是探讨脑卒中伴2型糖尿病(T2DM)患者应用磁共振成像(MRI)评价颈动脉粥样硬化斑块的价值。选取脑卒中伴T2DM患者81例(观察组),脑卒中不伴T2DM患者90例(对照组),比较两组颈动脉斑块MRI分型等差异。观察组颈动脉斑块MRI分型Ⅳ~Ⅴ型、Ⅵ型比例,血管总面积(TVA)、管壁面积(WA)和管壁标准化指数(NWI)、颈动脉狭窄程度、纤维蛋白原(Fib)明显高于对照组(P<0.05),而Ⅲ型比例明显低于对照组(P<0.05)。颈动脉狭窄程度与TVA、WA、Fib呈正相关(P<0.05)。MRI在斑块成分分析、颈动脉血管壁定量测量方面有较好的应用价值,特别是应用于脑卒中伴T2DM患者颈动脉粥样硬化程度较重时。

Abstract: This study explored stroke patients with type 2 diabetes mellitus (T2DM), the value of magnetic resonance imaging (MRI) in evaluating carotid atherosclerotic plaque. 81 stroke patients with T2DM (observation group) and 90 stroke patients without T2DM (control group) were selected, the MRI Classification of carotid plaque and so on between the two groups were compared. The proportion of Carotid Plaque MRI types Ⅳ-Ⅴ and Ⅵ, total vessel area (TVA), wall area (WA), Normal Wall Index (NWI), degree of carotid stenosis and fibrinogen (Fib) in the observation group were significantly higher than those in the control group (P<0.05), the proportion of type Ⅲ was significantly lower than that in the control group (P<0.05). MRI has good application value in the analysis of plaque composition and quantitative measurement of carotid artery wall, especially when the carotid atherosclerosis of stroke patients with T2DM is severe.
