Abstract: This study explored the application value of voxel incoherent motion (IVIM) in the diagnosis of renal transplantation rejection. 53 patients with renal transplantation were selected as the research object. According to the pathological results, there were 24 patients with acute rejection (rejection group) and 29 patients with normal renal function (non-rejection group), and analyzed the difference of IVIM parameters between rejection group and non-rejection group. The total apparent diffusion coefficients (ADCT) of renal cortex and medulla in the rejection group were significantly lower than those in the non-rejection group (P<0.05). There were no significant differences in the pure diffusion apparent diffusion coefficient (ADCD), pseudo perfusion apparent diffusion coefficient (ADCP) and volume fraction of pseudo diffusion (FP) in renal cortex and medulla between the rejection group and non-rejection group (P>0.05). There were no significant differences in IVIM parameters between patients with cellular rejection, humoral rejection and mixed rejection (P>0.05). The areas under the ROC curve of renal cortex and medulla ADCT in the diagnosis of acute rejection of transplanted kidney patients were 0.793 and 0.889, respectively, P<0.05. The total apparent diffusion coefficient of IVIM scan parameter has a good application value in the diagnosis of renal transplantation rejection.
