X线、MRI在儿童肱骨外髁骨折诊断中的价值研究

The Value of X-ray and MRI in the Diagnosis of Lateral Condylar Fracture of Humerus in Children

摘要: 本研究探讨儿童肱骨外髁骨折诊断中,应用X线、磁共振成像(MRI)的价值。选取急性肘关节外伤儿童94例,给予X线、MRI检查,分析两者诊断肱骨外髁骨折诊断的价值。94例急性肘关节外伤儿童中,确诊肱骨外髁骨折74例;MRI诊断肱骨外髁骨折的灵敏性、准确性和阴性预测值均为100.00%,明显高于X线检查的83.78%、85.11%和60.00%(P<0.05);MRI诊断肱骨滑车软骨链完整性的灵敏性、准确性、阳性预测值和阴性预测值分别为93.02%、96.81%、100.00%和94.44%,明显高于X线的62.79%、78.72%、87.10%和74.60%(P<0.05);MRI冠状位3D-FS-FSPGR/3D-FSPGR序列诊断肱骨外髁骨折的灵敏性、准确性和阴性预测值均为100.00%,明显高于FS-PDWI、FS-T2WI序列(P<0.05)。相比较X线,MRI在儿童肱骨外髁骨折诊断中有较好的诊断价值,值得临床使用。

Abstract: This research investigated the value of X-ray and magnetic resonance imaging (MRI) in the diagnosis of lateral condylar fracture of humerus in children. A total of 94 children with acute elbow trauma were selected and given X-ray and MRI examination, the diagnostic value of both methods in the diagnosis of humeral lateral condyle fracture were analyzed. Among 94 children with acute elbow injury, 74 cases were diagnosed as fracture of lateral condyle of humerus. The sensitivity, accuracy and negative predictive value of MRI in the diagnosis of humeral lateral condyle fracture were 100.00%, which were significantly higher than those of X-ray examination 83.78%, 85.11% and 60.00% (P<0.05). The sensitivity, accuracy, positive predictive value and negative predictive value of MRI in the diagnosis of humeral trochlear cartilage chain integrity were 93.02%, 96.81%, 100.00% and 94.44%, respectively, which were significantly higher than those of X-ray 62.79%, 78.72%, 87.10% and 74.60% (P<0.05). The sensitivity, accuracy and negative predictive value of coronal 3D-FS-FSPGR/3D-FSPGR sequences were 100.00%, which were significantly higher than those of FS-PDWI and FS-T2WI sequences (P<0.05). Compared with X-ray, MRI has better diagnostic value in the diagnosis of lateral condylar fracture of humerus in children, which is worthy of clinical use.
